Barthel Index score predicts mortality in elderly heart failure: a goal of comprehensive cardiac rehabilitation

چکیده

Abstract Background Accurate prediction of mortality in heart failure (HF) patients is crucial for decision-making regarding HF therapies, but a strategy the elderly has not been established. In addition, although favorable effects comprehensive cardiac rehabilitation (CR) on clinical outcomes and functional status have demonstrated, goal CR during hospitalization reducing remains unclear. Aims We examined whether assessment basic activities daily living (ADL) by Barthel Index (BI), most widely used tool ADL, useful predicting all-cause who received CR. Methods This study was single-center, retrospective observational study. retrospectively 413 aged ≥65 years (mean age, 78±7 years; 50% female) were admitted to our institute management hospitalization. Functional performing ADL ability assessed BI within 3 days before discharge. The endpoint death follow-up period. Results Of patients, 116 (28%) died period median 1.90-years (interquartile range, 1.20–3.23 years). an adjusted dose-dependent association analysis showed that hazard ratio (HR) increases almost linear fashion as score decreases corresponding 1.0 85 (Figure A). To minimize differences potential confounding factors between patient with low (<85) high (≥85), inverse probability treatment weighting (IPTW) calculated using propensity score. Kaplan-Meier survival curves, which selection bias minimized use IPTW confounders, had higher rate than did (≥85) B). multivariate Cox regression analyses, independently associated after adjustment predictors including brain natriuretic peptide prior (IPTW-adjusted HR, 1.75 [95% confidence interval, 1.03–2.98], p<0.001). Inclusion into model improved accuracy (continuous net reclassification improvement, 0.292, p=0.008; integrated discrimination 0.017, p=0.022). Conclusion A <85 at time discharge increased known prognostic markers, achievement ≥85 may contribute outcome patients. Barthel Index and Clinical Outcome in Elderly

Objectives: Among the elderly patients who are admitted in emergency departments, there is a sub-population of frail elderly who needs particular attention. The purpose of this study was to explore relationship between Barthel index and clinical outcome in elderly patients aged over 60 during a week follow up after initial admission. Methods & Materials: The method was a prospective cohort s...

Comprehensive rehabilitation in chronic heart failure.

Chronic heart failure (CHF) is a complex disease process connected with cardiovascular system as well as other organs and skeletal muscles. In connection with the above, cardiac rehabilitation, consisting of exercise training and diet supplementation, aims at recovery of physical, social and psychic function and removing risk factors influencing the occurrence of circulatory system diseases. Ev...

Multidimensional Prognostic Index based on a comprehensive geriatric assessment predicts short-term mortality in older patients with heart failure.

BACKGROUND Multidimensional impairment of older patients may influence the clinical outcome of diseases. The aim of this study was to evaluate whether a Multidimensional Prognostic Index (MPI) based on a comprehensive geriatric assessment predicts short-term mortality in older patients with heart failure. METHODS AND RESULTS In this prospective study with a 1-month follow-up, 376 patients age...

Cardiac Rehabilitation in Patients with Heart Failure.

UNLABELLED Reduced exercise capacity negatively affects the ability of patients with heart failure (HF) to perform activities required for daily life, further decreasing their independence and quality of life (QoL). Cardiac rehabilitation (CR) can effectively improve aerobic fitness and overall health status in patients with HF. Low referral rate is an important limitation that may impede succe...

High-sensitivity cardiac troponin T predicts non-cardiac mortality in heart failure.

BACKGROUND  Cardiac troponins are independent predictors of cardiac mortality in patients with heart failure (HF). Recently, elevation of troponins was described in non-cardiac diseases such as stroke and infection, among others, but it remains unclear whether high-sensitivity troponin T (hs-TnT) predicts non-cardiac mortality in HF patients. METHODS AND RESULTS  Four-hundred and forty-four c...
